Submit an inquiry online

The first step to filing a complaint is to fill out our preliminary inquiry form online. After you complete our online form, the MCCR Intake Unit starts the Intake Process.

Important information about filing a complaint

  • Employment discrimination complaint ​must be filed within 300 days of the alleged unlawful incident
  • Housing discrimination complaints must be filed within one (1) year of the alleged unlawful incident
  • Public accommodations, state contracts, health services, and commercial leasing discrimination complaints must be filed within six (6) months of the alleged unlawful incident

If you are experiencing trouble submitting your inquiry via the online form, you can complete one of the below forms instead. 

To submit any of these forms, you may: 

  • Complete the form on your computer, save it, and email it as an attachment to [email protected] with “Preliminary Questionnaire” in the subject line;
  • Print the form, complete it, and fax it to 410-333-1841; or
  • Print the form and mail it to:
